We are recruiting Band 5 Community Nurses to help us deliver the best experience possible in South Glos, South Bristol and North Somerset. As the South West's largest provider in community healthcare, our multi-disciplinary teams of nurses, therapists, clinicians and support colleagues help people remain safe and live as independently as possible after injury, illness or hospital admission. In this role, you will focus on collaborative, safe, effective and evidenced-based practice. Working closely with primary and community services as well as with social care partners, you play a critical role in ensuring a person-centred experience. You Will Assess, plan, implement and evaluate individual, patient-centred interventions drawing on a wide range of specialist knowledge gained from practical experience and specific training. Be competent in a broad range of clinical skills including catheterisation, pain management, wound care, end of life care and IV medication after specific training and more. Actively use and promote shared decision-making, empowering people to improve self-management of their long-term condition while working collaboratively with our teams and system partners Supervise colleagues including; peers, students and unregistered workforce in nursing competencies, helping us shape our existing service and contribute to the future design and delivery.
