Interviews will be held on 14/10/2024. An exciting opportunity has become available for a Band 5 Well-being Practitioner to work within our Carmarthenshire Primary Mental Health Service. Individuals will be expected to be enthusiastic and committed to provide well-being and mental health support to help address locally identified need and sustain capacity within wider Primary Care Mental Health Services. Well-being Practitioners will deliver timely interventions in the community which will ensure early intervention, preventing many individuals from requiring secondary mental health services. The ability to travel between sites within the county in a timely manner is essential. The post holder will use a range of communication methods adaptable to the needs of individuals, families/carers and partner organisations to support individuals living with symptoms of mild mental illness. The post holder will provide support to the Service at large to enable the best possible care outcomes for those experiencing mental health distress and connect individuals with local Mental Health and well-being services where appropriate, including liaising with partner organisations such as GP's, Primary Care, WAST, A&E, Police and Third Sector etc. Provide general mental health advice, relating to clinical risk, Mental Health Act or Mental Capacity Act. Prioritise own workload to ensure that all individuals receive a timely intervention. Develop and maintain relationships with Health Board services and partner agencies such as GP's, 111 Option 2, Police, Third Sector, Local Authority and WAST etc. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
