Our Community Transfer of Care Hub (CToCH) service is looking for an enthusiastic coordinator to support the clinical and admin teams. There is a focus on data collection to evidence the difference new ways of working are having on flow through the system, bed days saved and improved patient experience. The post will require you to work in close collaboration with colleagues from acute hospitals, social care, primary care, Pathway 2 home providers and our Integrated Care Board (ICB) and really make a difference for these service users and their families. We are developing new ways of working alongside acute hospital and local authority colleagues to support the service users admitted to acute hospital beds to leave hospital in a safe and timely way and maximise capacity of community resources. The Service currently operates 08:00 - 18:00 7 days a week including Bank Holidays. This role supports the delivery of the admin component within the Community Transfer of Care Hub (CToCH). The role will be based between Castlewood, Clevedon and South Bristol Community Hospital, Hengrove. Organises own time and is clear and transparent in day to day working. Leads on reallocation of work during Annual Leave and Sickness and ensuring the service continues to run effectively. Liaises with other professionals, departments, services as required Identifies issues with processes, creates solutions and implements changes in conjunction with the wider team. Co-ordinates, monitors and supervises admin staff employed within the service, and their activity reporting any concerns to the Management Team as necessary. Leads on developing working procedures and guidance for administrative team tasks. Responsible for ensuring the workload within the administrative team is delivered efficiently, accurately and within set timeframes. Liaises closely with all members of the administrative team, providing leadership and direction. Direct management, including first level sickness management, grievances and discipline, appraisals, allocation of work, training and development. Undertake effective employee appraisals for all managed administrative staff, identifying and agreeing individual training needs and personal development plans in line with service objectives. Take responsibility for own role, identifying any knowledge/skills gaps where additional support/training is needed.
