This is an excellent opportunity for an experienced clinical pharmacist to lead, develop and deliver anticoagulation services across Royal Brompton and Harefield Hospitals which includes the management of the outpatient anticoagulation clinics as well as working closely with the Consultant Haematologist in supporting Trust initiatives and priorities for Venous Thromboembolism (VTE) and Hospital Acquired Thrombosis (HAT). This role will also involve leading a high-quality clinical pharmacy service to anticoagulation clinic and supporting the clinical pharmacy service to cardiothoracic surgery patients. You will be managing the Clinical Nurse Specialist for Anticoagulation and will work closely with the Consultant Haematologist and the surgical pharmacy team. The post holder will be required to work closely with the multidisciplinary teams (MDT), providing advice, support and education to patients referred to the anticoagulation clinic. You will need to have completed your post graduate Diploma in Clinical Pharmacy and you must be able to work under pressure and have a strong sense of accountability, possess excellent problem-solving and communication skills. The post holder will be responsible for leading and managing the outpatient anticoagulation service, working closely with other MDT members to provide specialist advice, dosing and correct monitoring for patients on oral anticoagulation. The post holder will be a key member and representative for the Royal Brompton and Harefield sites at the Trust wide Thrombosis and Thromboprophylaxis Committee meetings. The role will include developing and implements policies and guidelines to ensure safe and effective use of anticoagulants across the Trust. The post holder will also provide clinical pharmacy input to the cardiothoracic wards and clinics. "}
