An exciting opportunity has arisen for an enthusiastic and dedicated person to be part of the Integrated Supported Home Discharge Team (iSHDT). The team is multidisciplinary team consisting of Physiotherapists, Occupational Therapists, Social Workers, Social Care Assistants, Rehabilitation Assistants and Rehabilitation Practitioners . The team provide reablement support to patients in their own homes following hospital discharge. You will have support from and work closely with the multidisciplinary team to provide patient centred reablement and rehabilitation programmes. We are looking for an individual who has: Relevant experience in a health and social care setting and base level of Occupational therapy/Physiotherapy knowledge Knowledge of patient/client conditions relevant to community rehabilitation Strong administrative and communication skills, able to liaise with a wide range of stakeholders.
