Job Title: Nurse Associate Job Description Grade Band 4 Location Biz Space/SSPCN Practices/Community Venues Reporting to PCN Clinical Lead Nurse The post holder is a Nursing Associate registered with the Nursing and Midwifery Council. They are required to act within their professional boundaries, supporting and working alongside a multi-disciplinary team within a Primary Care Network (PCN) and general practice. The post holder will contribute to providing nursing care to patients registered with a GP practice within the South Sefton PCN. Working under the supervision of a Registered Nurse the post holder will focus on operating across specified areas, e.g. supporting clinics within the Women's Health Hub, assisting GPs, chaperoning duties, learning disability annual review and follow ups and health checks, The post holder will work with other members of the Multidisciplinary Team (MDT) to both organise and facilitate care appropriate to the individual needs of the patient. Work with GPs to support delivery and development of the Women's Health Hub. Carry out LD annual health checks, ongoing reviews and follow ups. Qualified to deliver cervical screening, or willingness to train. Working across community, GP practices and home visits within the South Sefton PCN. "}
