The role of the discharge coordinator is to coordinate and facilitate safe and effective discharge, working alongside ward staff/ therapy teams/ discharge hub/case managers. Gathering patient social information on admission, referring to discharge hub in a timely manner when patients are medically stable for discharge. Liaising with family/friend carers to discuss any concerns they may have for discharge. The successful candidate will raise discharge concerns to the right teams as concerns arise.Use clinical systems, ie ICE, Care flow connect, care flow live. Pro-track. You will have a full understanding of all the Discharge pathways. and havea full understand of the covid policy for patients returning to care home and the need to ensure swab in date and discharge hub complete the correct risk assessment. You will gather accurate information for the site matron of expected discharges on ward daily and attend board rounds daily. Excellent communication skills are required for this role. "}
