The Care Navigator Team is embedded within the Children & Young People's Mental Health Service (CYPMHS) Wirral 0-18 team who work with young people presenting with moderate to severe mental health needs. The work is mostly community based so having access to transport is essential Meeting with families and young people Attending meetings with other professionals Developing plans to access community support Developing sound knowledge of community services Supporting young people to access community services Raising concerns with team leads if presentation of young people deteriorate CWP provides health and care services for local people, including mental health, learning disability, community physical health and all-age disability care – including the provision of three GP surgeries.
