The post holder will have a desire to develop their skills in working with adults with severe and enduring mental illness, some of whom will have not had previous access to psychologically informed interventions. The post-holder will work collaboratively with service users, their families, carers, and multi-disciplinary team members to provide assessment, care planning and coordination of care, supporting collaborative decision-making about care and treatment. The successful candidate will be part of the core team and duties will also include, holding a small caseload, completing Initial Assessments, Care Planning, Risk Assessing, being part of the Duty Team on a rota basis and will contribute to the implementation of the LIPP (Low Intensity Psychosis Pathway) Service Mini Team. The post holder will support the delivery of a set of wellbeing-focused psychologically informed interventions, aligned to cognitive-behavioural principles, based on the best evidence available, that address problems often experienced by people with severe mental health problems under the supervision of a clinical/counselling psychologist or CBT therapist, where this is identified in personalised care planning and support. The Successful Candidate Will hold a graduate certificate (Level 6) or postgraduate certificate (Level 7) delivering the national curriculum for Mental Health and Wellbeing Practitioners (Adult Specialist Mental Health) work closely with colleagues in a multi-disciplinary team to deliver interventions for our service users carry out relevant risk assessments and risk management with the multi-disciplinary team engage with and gather information from service users, relatives, and patient records to develop a formulation work in collaboration with the individual service user and their family and carers as appropriate to develop care plans that are focused on strengths and are outcome based.
