The fixed term is for an initial period of 6 months extendable to 12 months. We are pleased to announce a unique and highly rewarding opportunity for a resident doctor at ST3+ level in Higher Specialist Training to join the Hospital at Home (H@H) and Same Day Emergency Care (SDEC) services at Darent Valley Hospital. This newly created post is part of a pioneering cross-sector expansion initiative designed to enhance our established care pathways. By joining our team, you will contribute to shaping the future of innovative healthcare delivery. The aim of SDEC, H@H and virtual wards is to achieve a reduction of avoidable hospital admissions and to facilitate earlier, safe discharge of patients from the acute services. Further to this, there is substantial opportunity to work with the clinical lead to identify additional service development, lead on change and provide leadership for junior colleagues and the acute teams. Job plans are designed to facilitate this. A full-time job plan with working hours from 8.30 – 16.30. You will be expected to triage referrals, review patients on the take that may be managed directly under Hospital at Home rather than being admitted to an acute bed, and supporting the step down of patients from the ward. You will be involved in reviewing patients as well as formulating management plans for patients under hospital at Home. You will occasionally accompany nursing staff to visit patients in their own home. You will work seamlessly between SDEC and HAH. We expect the post-holder to be involved in the development of medical students, trainees and allied health professionals.
