As part of Pathways to Recovery and the Community Mental Health Hubs, Social prescribers are integrated into a wider partnership of delivery, including voluntary sector, local authority and health providers. The post holder will support individuals to build independence and personal resilience, enabling self –management of their health and wellbeing. This will be achieved through the creation of Co-produced Action Plans which will be tailored to individual need. Key to the role will be the ability to build trusting relationships quickly whilst motivating individuals to make sustained changes, working sensitively, and overcoming barriers to access. It is important to understand that you will will be working with people who may have serious mental illness. Social Prescribers will build and develop relationships with a wide range of services and activities in order to address social and health care needs, signposting and offering support to individuals to participate and benefit from community-based activity. 35 hours per week. We anticipate that this role will be predominantly based at the community mental health hub with regular delivery in the community and at Foss Park Hospital. As the Hubs model develops, you may also be required to work out of community mental health hubs in Acomb and the Tang Hall/ Burnholme areas.
