Ensure due regard to patient confidentiality the coordinator needs to interpret and share the information that is necessary to promote integrated care across organisations. The post holder will be responsible for co-ordinating integrated care delivered through the Integrated Care Team (ICT). The post holder will support the delivery of Integrated Care and influence service development. The post holder must have an ability to operate within complex health and social Care system, across Health and Social Care and third sector providers. The post holder provides leadership within Integrated Care services. The post holder will teach and present to groups of staff, changes in both integrated care provision and direction and ensure change in policy is implemented across the whole system. Coordination of interventions for patients, communication with GP practices and collaboration with staff inside and outside the Integrated Care Team (ICT).
