Main duties of the job Home First supports the approach of \"why not home, why not today?\" Wherever possible patients should be supported home for ongoing assessments where this is not possible alternative pathways are available. Staying in hospital for longer than necessary has a negative impact on outcomes and ensuring patients are given the chance to continue their lives at home is vital for their long-term wellbeing. @home works across organisational boundaries and as an integral part of the integrated health and care team is responsible for providing rapid access, enhanced, short term health and care support for people who would otherwise need emergency hospital admission as well for as those medically stable to return home from an acute hospital but requiring enhanced support as one stage in their on-going care pathway. The @home service spans the acute and community providing joined up, co-ordinated care to local citizens with health and social care needs. The Recovery Athome Team will form an integral and essential part of Discharge to Assess, providing timely care and support to facilitate discharge from hospital and onward assessment and identification of need. Perform patient assessment (under supervision and after appropriate delegation from the registered practitioner) plan and delivery high standards of care.
