London North West Hospitals NHS Trust is seeking to appoint a Trust Grade Specialist Registrar Doctor for 12 months in the Department of Medicine for Older People at Northwick Park & Ealing Hospital to work on our Elderly care wards which focuses on a comprehensive geriatric assessment, and early discharge planning. Our goal is to provide a continuum of care for Older People spanning the acute District General Hospital, the local Specialist Rehabilitation Hospital and the necessary medical, social and therapies provision to promote maximal independence of older people living in the community. We aim to treat acute illness appropriately, to enhance quality of life, to reduce disability, recognise when life is nearing its end, and to provide individualised person-centred care in the right setting at the right time. Duties The post offers the applicant an exciting opportunity to gain experience in Care of the Elderly. The main duties of include: Comprehensive Geriatric Assessment of patients as part of a multidisciplinary team Providing the necessary communication back to GPs and family members Supervising the junior members of the medical team in day-to-day management of patients on the elderly-care wards Being involved in decision-making regarding care arrangements for older patients which may include case conferencing, best interest meetings and family meetings Making presentations at meetings, and undertake audit and other clinical governance activities.
