The case load will consist of a wide range of clinical conditions including frailty and older people with multiple long-term, complex conditions. The Dietitian will assess and treat patients in community settings including care homes and patients own homes, with the main focus on malnutrition. You will work as an autonomous practitioner and alongside community multidisciplinary teams including Intermediate Care, Community Nursing, Health and Social Care teams, and Care Homes. The Dietitian will be the nutrition expert within those teams, being a highly skilled Dietitian with a dynamic, self-motivated and innovative approach to your work. You will be passionate about patient care, with excellent communication and engagement skills, team working and strong time management. The job is based at a number of community locations including Teignmouth, Torquay (Community Dietetics base), as well as working from home. You must be able to travel independently and have access to a car on a daily basis. Working pattern to include Wednesday (for weekly triage/clinical supervision with the Community Dietetic team). The post is a 0.5 WTE (18.75hours/week) fixed term contract for 12 months maternity cover, with possible extension/permanent hours depending on movement within the team. You will provide a Dietetic service to adult patients across the community services in Torbay and South Devon, with the main focus on malnutrition and oral nutritional support. Dietetic assessment and management of patients living in the community (own homes and care homes). You will work alongside and support health and social care professionals within community MDTs. To attend regular team meetings to discuss the nutritional care of patients, as part of their multi-disciplinary care. Promote and contribute to the wider development of the dietetic service and nutritional provision within Torbay and South Devon community services. You will work alongside community health and social care multidisciplinary teams whilst maintaining regular contact with the Community Dietetics team for professional and clinical support. Intermediate Care is a time limited, community based service which aims to: To help people avoid going in to hospital unnecessarily To help people be as independent as possible after a stay in hospital or after a period of ill health To prevent people from having to move in to a residential home until they really need to The Community Clinical team of Dietitians are based at Torbay Hospital and are part of the wider dietetic team including Acute, Diabetes, Paediatrics, Lifestyles, and Cancer Services. The team has over 40 staff members – dietitians, dietetic support workers and admin support. The Community Clinical Dietitians are a friendly, supportive, proactive and professional team, which has increased in size over recent years following investment in a number of areas. You will have full support from the Clinical Community Dietetic Team Lead, IC Leads, Community Health and Social Care Services Manager as well as the existing Community Clinical Team of Dietitians and Specialist Secondary Care Dietitians.
