The Patient Pathway Co-ordinator will be responsible for ensuring the efficient management of patient journeys through the service, from initial referral to the conclusion of treatment. The role involves working closely with clinical teams, families, and young people, ensuring that administrative processes are streamlined and that patients receive timely access to the appropriate services. This role is critical in ensuring the service runs smoothly, providing administrative support, tracking progress, and managing appointments. The successful candidate will be someone with experience working within a multidisciplinary team in a healthcare or social care environment, with a passion for supporting young people. A commitment to working in an inclusive and empathetic manner is essential. You should have excellent communication skills, a proactive approach to problem-solving, and the ability to work efficiently in a high-pressure environment. Candidates must have a good general level of education, with English and Maths to GCSE standard or equivalent. This is a full-time position, 37.5 hours per week, Monday to Friday. We encourage interested candidates to apply as soon as possible The Children and Young People’s Gender Service is delivered by a comprehensive, multidisciplinary team, including paediatricians, mental health practitioners, youth workers, nurses, speech and language therapists, and occupational therapists. The service provides an in-depth assessment of each child or young person’s needs, culminating in a personalized care plan developed collaboratively with them and, when appropriate, their families or supporters. A range of care interventions is offered to address mental, physical, emotional, psychological, and social needs. The post holder will play a key role in actively managing the administrative processes within the patient care pathway. They will identify and progress the next steps in line with clinical needs, ensuring a smooth and efficient journey for patients. This includes developing an understanding of the pathway milestones and providing comprehensive administrative support to the multidisciplinary team in their care of children and young people. Following the recommendations of the Cass Review, NHS England has developed a new clinical model for gender identity services for children and young people, emphasising regional delivery through multidisciplinary care centres. The London regional centre, based in London, is delivered by a partnership of Evelina London Children’s Hospital (part of Guy’s and St Thomas’ NHS Foundation Trust), Great Ormond Street Hospital, and the South London and Maudsley NHS Foundation Trust. The North West regional centre, based in Manchester and Liverpool, is delivered by a partnership of Alder Hey Children’s NHS Foundation Trust and the Royal Manchester Children’s Hospital (part of Manchester University NHS Foundation Trust). As of 2024, the South West regional centre, based at University Hospital Bristol and Weston NHS Foundation Trust, also went live, further strengthening the national network of hubs. These centres work together to provide high-quality, timely care to young people across the UK, including addressing the national waiting list.
