The base for this Team is Central Park in Manchester and supports children and young people across the ten boroughs of Greater Manchester. There will be an expectation for the Peer Support Navigator to independently travel across Greater Manchester. The Greater Manchester Children’s and Young People’s Positive Health Pathways Team is part of a Greater Manchester wide multi-disciplinary pathway working collaboratively with the children and young people in contact with the criminal justice system, SARC and secure welfare provision. Through a trauma informed and biopsychosocial model of care the service facilitates assessment; joint care planning; intervention and evaluation of outcomes. The team aims to support the children and young people in understanding and making informed decisions about their health and wellbeing, offering access to opportunities that can contribute to breaking the cycle of offending and building resilience. Role The role of the Peer Support Navigator is to draw on their lived experience to support children and young people to enhance their health, wellbeing, and quality of life by assisting children and young people to connect with and access health services, voluntary sector services and community-based activities and support. Through sharing wisdom informed by experience and knowledge the post holder will: Support and promote social prescribing as a user led concept with a playful, accepting, curious and empathic approach. Emphasise hope and optimism, setting and inspiring goals with the child and young person. Value experiential learning. Focus on strengths rather than deficits taking a child focussed and centred approach. Taking a trauma-informed approach to care. Age-appropriate open communication adapting to the child’s age, vulnerability, health needs, diversity, and protective characteristics. Creating a safe space for ideas to be discussed.
