The patient care coordinator will also be responsible for ensuring that patient records in either CRM System or Patient Admin System created by them and for patients whom they subsequently take responsibility for are accurate and updated routinely. In line with the patient journey the PCC will ensure that the process is followed and that all appointments are made and completed in a timely fashion and that all financial transactions and processes are followed accurately. Duties Serve as the primary point of contact for patients, addressing enquiries and providing information regarding services. A bonus scheme will be discussed based on the successful conversion rate from enquiry to booking. Schedule and confirm patient appointments, ensuring efficient use of resources and minimising wait times. Maintain accurate patient records and update information in the office management system as required. Collaborate with healthcare professionals to ensure comprehensive patient care and follow-up on treatment plans. Assist in the preparation of patient documentation and facilitate the completion of necessary forms. Handle administrative tasks such as managing correspondence, filing, and maintaining office supplies. Ensure compliance with health regulations and maintain confidentiality of patient information at all times. Skills Strong organisational skills with the ability to manage multiple tasks effectively. Excellent communication skills, both verbal and written, to interact with patients and staff professionally. Proficiency in office management software and basic computer applications. A compassionate approach to patient care, demonstrating empathy and understanding in all interactions. Attention to detail to ensure accuracy in patient records and documentation. Ability to work collaboratively within a team environment while also being self-motivated.
