The team provides person -centred, community-based, mental health support and recovery to a vibrant and diverse population in the London Borough of Haringey. We offer a range of interventions across health and social care including; Peer Supported Open Dialogue and Trauma Informed Psychological Therapy. We are supported by a team based Employment Support (IPS model) Worker, Dual Diagnosis Worker, Link Worker, Housing and Financial Advisors, Family Therapist, Psychiatrists, Psychologists, Community Mental Health Nurses Social Workers, Occupational Therapist and Support Workers. Your work in the team will be informed by the principles of Enablement, Recovery and Social Inclusion in the team and among our stake holders. You are required to have a Degree or equivalent in Nursing. with experience working in community mental health services. A commitment to this service user group and to the multi-disciplinary team of nursing, medical, psychological, employment and social work professionals and a sound working knowledge of Enablement and Recovery is required. Leadership, training and development opportunities are offered. Closing date for applications: 15th January 2025 Interviews will be held in 22nd January 2025 Carry overall responsibility for the management of a caseload, acting as a Key Clinician ( Care Coordinator) Deliver effective care and treatment that is carried out within a multidisciplinary framework and in collaboration with other agencies. Involve service users and their carers in planning care that includes all of their individual and diverse needs. Design individual programmes of care collaboratively with patients to maximize self- management and social inclusion and reflect their own goals and aspirations. Review and adjust plans according to changing service user needs. Provide evidence based therapeutic interventions using developed interpersonal skills such as CBT or psycho-education. Provide service users and their carers with clear and intelligible information which will assist them in making informed decisions about their treatment and care. Administer medication, monitor for effects and side-effects, in accordance with NMC Standards and Trust policies.
