CAP is hosted by York and Scarborough Teaching Hospitals NHS Foundation Trust and is an integral part of the Humber and North Yorkshire Integrated Care System. As a system we are embarking on major transformational change across our urgent care, elective care and diagnostic services. As a Project Manager you will work closely with the clinical and operational team members across the Trust, leading on pan-Trust projects to assist with driving and facilitating change across clinical pathways to ensure the best and timely care for our patients. The project roles will support our clinical networks, with opportunities across elective services and diagnostic networks. The Humber and North Yorkshire Collaboration of Acute Providers (CAP) delivers programmes of work at scale to support improved patient care. Working across the acute trusts within the provider collaborative, the post holder will be responsible for the overall management/leadership and delivery of specified high impact projects within the Diagnostic work programmes. The post holder will lead the development of the project plan, monitoring and delivery of actions, risks and issues relates. Working closely with clinical leads through clinical networks and with operational managers across the acute trusts in Humber and North Yorkshire, the local Integrated Care System (ICS) and regional colleagues, this post will work will support our ambitions on productivity, improving pathways for patients and developing and implementing new ways of working across clinical and operational teams. The post holder will co-ordinate the work of associated workstreams within the project, monitoring the progress of their work against relevant work stream Project Plans and link into completion of the Programme.
