To undertake non-complex clinical procedures under the guidance of senior team members e.g. venepuncture, patient specific sub-cutaneous injections, sample collection, blood pressure monitoring, medications, wound care, Catheter care and other tasks according to training and competencies. To recognise their responsibility and accountability in accepting the delegated task. Empowering patients to regain their health and independence in collaboration with other members of the Multi professional Team. Liaising with clients, colleagues, family and other professionals in ensuring integrated quality care is provided. To keep accurate, timely records according to Trust policy. To escalate and report patient progress/ status on a shift by shift basis to senior team members To demonstrate flexibility and adaptability working across community teams according to service needs. To commit to undertaking training and education to progress clinical skills as indicated by service development needs To respond effectively and actively to complaints and escalate appropriately following departmental policies. To assist registered staff in care and treatment and participate in the review process.
