Providing nursing care to patients, of the Blaenau Gwent area, in their own homes, working collaboratively with the MDT to support a seamless high quality care provision to the patients we support in the community. Supporting the strategic direction and models of service delivery working across PCCS. Main duties of the job Clinical Skills To support patients clinical needs within the community a diverse clinical skill set is required. These include: diabetic management, palliative and end of life care, complex wound care, care/management of a variety of surgical drains, pressure area care/management, bowel care/management, catheter care/management, dietary care/management, health promotion, equipment provision/review, venepuncture, IV therapy, enteral feeding and gastrostomy tube care/management, tracheostomy care/management, ability to undertake verification of expected death, (or willing to work towards). Qualifications Essential criteria Registered Nurse (with current NMC registration). Evidence of continuing Professional development. Evidence of Portfolio Development Desirable criteria Diploma Level Education.
