We have a vacancy within our new Partnership Hub for a Patient Care Coordinator. You will play a key role in delivering our patient experience strategy, ensuring our customers have a first-class experience and receive the care and support they need. Role: Patient Care Coordinator – 1-year fixed term contract, full time Location: Remote Salary: £27,300 per annum Hours: 35 hours per week, working on a shift pattern, Monday to Friday between 8am and 6pm. We work on a rota basis between 8.00am and 4.00pm, 9.00am and 5.00pm and 10.00am and 6.00pm. As a Patient Care Coordinator, you will: Facilitate the delivery of outstanding care by being responsible for the co-ordination of administrative tasks and processes, ensuring these are managed in accordance with targets and agreed timescales. Be responsible for providing a high standard of professional and patient focused services across a range of specialities and modalities. Ensure patient records are maintained and that all correspondence, reports and results are uploaded correctly and in a timely manner. Track patients in accordance with RTT and DM01 guidance and standards. The ideal candidate will have: Excellent empathy, communication skills and exceptional attention to detail. Prior experience in a customer service role, ideally within a healthcare setting. Knowledge and understanding of RTT and DM01 reporting requirements. Ability to remain calm and professional in a very fast-paced, constantly evolving environment. Strong organisational skills and attention to detail. An ability to cope with the sensitive nature of work, and to deal with challenging and difficult behaviour in an appropriate manner.
