Assessing and commissioning packages of care and residential and nursing placements Hospital discharge planning Safeguarding enquiries Court of protection work Complete assessment visits - assess patients/service users needs and identify support required to support their discharge from hospital. To follow relevant legislation and law - Care Act 2014, Human Rights Act 1998 and Mental Capacity Act 2005. To complete Care Act assessments, support plans, mental capacity assessments and best interest decisions and Transfer of Care documents.
