Clinical Lead Patient Flow Hub - Band 8a Fixed Term Contract - 6 months - Full Time Chippenham Community Hospital Due to the upcoming departure of the current clinical lead, we are looking to recruit on a fixed term 6 month basis initially. It is possible this may become a substantive role at the end of the fixed term period. Wiltshire Health and Care provide community services across the county, both within our Community Hospital sites and within patients own homes. To ensure patients receive care in the most appropriate environment, and maintain flow between WHC, acute trusts and social care services, the flow hub play a crucial role in directing patients through the correct pathway of care. We are currently looking to recruit an experienced clinician either a Nurse or AHP who holds advanced skills in assessing complex patients and advising the appropriate pathway for their ongoing care. The successful candidate will use their excellent communication skills to forge strong relationships both within Wiltshire Health and Care and with external organisations. They will play a pivotal part in representing WHC in system discussions and operational working groups supporting discharge and flow and ensuring that effective care transfers for patients are facilitated. A key element of the role would be to support an improved patient experience in their journey between care settings, supporting the right care in the right place at the right time. Our Flow Hub Clinical Lead would be accountable to Head of Operations for In Patients, MIU and Flow, and work in partnership with both WHC services and the wider health and social care community. "}
