The post would suit an enthusiastic nurse practitioner, valuing the opportunity to work collaboratively with members of a well-established multi-disciplinary team, where physical health is a core service within the hospital. The successful applicant will work closely alongside another physical healthcare practitioner as well as other nursing colleagues within the team. The team also works alongside subcontracted specialists within the department including, GP services, Podiatry, Dentistry and Opticians as well as our internal AHP professions, e.g. Physiotherapist & Dieticians. The physical healthcare practitioner will have the opportunity to gain experience working with a diverse population that includes male and female mental health, learning disability, autism and older adults, including those who may have experienced trauma and/ or neurological difficulties. TEWV NHS Foundation Trust are offering an exciting opportunity for an innovative and dynamic individual to join our dedicated team to clinically lead (in collaboration with the physical healthcare team and medical colleagues) on the provision of physical healthcare for inpatients in Secure Forensic services. As a Physical Healthcare Practitioner, you will have a key role in supporting inpatient teams by working as part of the Multi-Disciplinary team to develop and implement treatment and care plans and to support service improvement within this area of practice.You will work closely with the clinical teams to provide a high quality service to patients through support, intervention and clinical expertise on the various aspects of physical health including: assessment, diagnosis and review, chronic disease management, management of acute presentations, palliative and end of life care and ongoing physical health monitoring.The role also involves collaborative working with the Trust wide Lead for Physical Health on various physical health work-streams, and the delivery of staff training, education and development. You will support evidence based practice and have experience of working independently and autonomously in a Nurse Practitioner role, utilising clinical assessment skills and demonstrating sound clinical decision making. You will have oversight of a patient caseload, whilst supporting the strategic role of developing physical healthcare across the Trust.
