This job advert is recruiting to positions within the East locality and based in Wallisdown, Poole, however you may be required to provide support across both sites as necessary. There will also be a requirement to make off-site visits to undertake assessments and attend meetings and it is therefore essential that you have a full driving licence with access to a vehicle for business use. Working within the remit of the National Frameworks for Continuing Healthcare, the post holder will deliver an effective and competent level of service and consistently deliver a client-focused service. The post holder will be required to manage a caseload of CHC Eligible individuals ensuring the National Framework for NHS Continuing Healthcare and Funded-Nursing Care is implemented and complied with, according to the principles and processes. This caseload of complex cases would require annual eligibility reviews, ensuring value for money placements which meet the needs of the individual and identify opportunities for development and improvement to assist delivery of best practice healthcare. The post holder will assist NHS Dorset commissioners in making recommendations on the most appropriate placement. NHS Dorset also offer this role as a development post whereby employment would be commenced as a Band 5 and a development programme would be undertaken to enable advancement to a Band 6 Senior Practitioner role. Although this is not a patient facing role, and is predominantly desk based, requiring high levels of concentration and an ability to analyse health, social care, and other agency records to identify whether the individual has/had a Primary Health Care Need, visits to patients and offices is a frequent requirement of the job .
