Taking responsibility for a case load of patients and initial comprehensive assessments, delivering diagnosis and formulating treatment plans and on-going review. An opportunity has arisen for a mental health nurse to join us as a B6 MA Nurse in Central Memory Assessment Service (MAS). This is a long established multi-disciplinary memory assessment service providing a service of excellence across Conwy and Denbighshire. We will provide you with the opportunity to develop transferrable skills - to become specialised in the assessment process by further developing your assessment techniques whilst carrying out in-depth first line assessments; collecting and collating information in an appropriate format to present to your MDT colleagues; sharing and discussing sensitive information as well as participating in the diagnostic process; providing knowledge and support during those early days of diagnosis as well as being involved with initial therapeutic group/ individual work. The role will require a mix of nursing skills from a range of professional specialist areas - Primary Care, Liaison, Adult Mental Health and Older Person's Mental Health.
