The role involves supporting learning disabled adults who require specialist health support due to their complex physical, sensory, communication and behavioural impairments. The post is based within East North East Community Learning Disability Team (CLDT) working alongside and supporting the members of the full multi-disciplinary team - band 4 senior support workers, dietitians, nurses, occupational therapists, physiotherapists, psychologists. psychiatrists and speech and language therapists. The post works across a variety of settings including the CLDT office, service users' homes, day placements as well as within a therapy base offering hydrotherapy, rebound therapy and therapy kitchen. The issue of a laptop and mobile phone also enables remote and hybrid working. Training will be offered under all clinical areas/arenas to support development into this role. - Under the guidance and supervision of the teams' clinicians to carry a caseload taking responsibility for implementing and reinforcing health management care plans using a range of nursing and therapeutic techniques. - To participate in health assessments and monitoring through pre-set directed observations and information gathering, liaising with service users and carers and other agencies. - To provide and monitor the implementation of basic therapy equipment under the direction of a clinician. - To support the checking, managing and session planning of the hydrotherapy and rebound specialist environments collaborating with the physiotherapists and senior support workers About us To work under the guidance of community learning disability team clinicians to support service users and their carers around their complex varied health care needs. The overall responsibility for the service user always remains with the registered clinician. To support in the assessment of service users through information gathering through liaison with service users, carers and the MDT through observations and monitoring as guided by the registered clinician. To support, deliver and monitor therapy and nursing programmes within a prescribed framework to adults within the learning disability service who present with complex health and social care needs. To work independently in the community within set clinical parameters. To work alongside other therapy staff within the services specialist therapeutic hydrotherapy and rebound therapy suites. To liaise with service users, their careers, other health and social care professional and agencies within set clinical parameters/ To actively engage with supervision and appraisal process as identified in Trust Policy.
