They are a highly supportive team who are happy to develop an aspiring Band 5 nurse into a Band 6. They are also able to offer flexible working to provide a good work life balance. The community nursing team covers 8am to 8pm, 365 days a year. The shifts include a mixture of 8am to 4pm, 9am to 5pm and 12pm to 8pm. and 1 weekend in 3. We are also happy to consider part time hours. Within this role you will: Provide leadership and management to your team. Have responsibility for your caseload and have skills and competencies to provide excellent patient care in their homes. Undertake full nursing assessments of patients, including those with multiple co-morbidities and complex health and social needs. Be responsible for agreeing the patients care plan, liaising with others and adapting the care and treatment delivered according to need and patient choice. Work with the Integrated care system and MDT to provide holistic care for your patients. Responsible for contributing to the holistic assessment, planning delivery and evaluation of care to patients within their own homes. implement, monitor and maintain high standards of care delivery at all times. You will have a full UK driving license with access to a car for work purposes.
