It is an essential part of the Portsmouth Hospitals Stroke Pathway, ensuring the timely transfer of patients out of hospital for rehabilitation at home. The post holder will be working with a large but very closely working MDT with competencies across several disciplines: occupational therapy, physiotherapy, speech and language therapy and nursing. You willingness and ability to learn new skills as working across MDT is essential as well as excellent communication skills. You will be working mostly Monday to Friday 7.30/8am -15.30/16.00 plus providing some weekend cover on a rota basis. You will be carrying out but also progressing patient rehabilitation under indirect guidance. Experience of working with patients with neurological deficit and community working would be desirable. You need to be a confident car driver and user, as the team covers a large geographical area.
