We require an enthusiastic, innovative, and forward-thinking support worker/trainee (part time - 22.5 hours) to assist the team in all aspects of assessing, planning and implementing the delivery and review of patient care. In addition, you must be able to develop and maintain professional relationships with service users, carers, and other professionals. You will need to be flexible and adaptable in your approach to meet the needs of the service. A car driver will be essential to meet the needs of a community service. In return you will receive a full induction programme and supervision. Your role will be instrumental in developing the older adult service and providing excellent service to our older adults in the community and care homes. You will be involved in providing support for Older Adults living with Dementia in the community and care homes working alongside the multi-disciplinary team. You will be required to support other clinicians in the team to screen referrals, review notes and documentation, obtain updates from the care home and physical observations. Face to face reviews with patients and report on potential side effects of medications. Report any issues/ safeguarding concerns. The post holder will carry a small caseload and carry out basic physical health checks where necessary. You will provide support for patients and carers in care homes who have a diagnosis of dementia and mental illness. You will develop a sound knowledge of wider community services. We develop behaviour support plans alongside care homes and patients to help enhance the care homes knowledge to engage with patients when displaying sometimes challenging behaviours. We complete screenings for referrals using the PINCHME tool and discuss these collaboratively in the Multidisciplinary team meetings every Tuesday and Friday. You will take part in developing the behaviour support plans, screening of referrals and be part of the multidisciplinary team meetings. Our team works seven days a week, working 9- 9 shifts and 9- 5 shifts. "}
