The Team operates across Mid and North Hampshire to deliver acute hospital level care at home in response to urgent changes in health status for patients living with frailty. These vacancies are a direct result of our success and the recent expansion of the service through increased commissioning and a very successful GIRFT review. The successful applicants will be joining an established and supportive group of Consultant Practitioners from varied backgrounds. We welcome applications from all Allied Health Professions and Nursing. We are also able to consider applications from professionals who wish to develop into Consultant Practitioner roles. Due to the expansion of the Hospital at home frailty Virtual Ward (H@H), we are seeking a consultant practitioner to work alongside the existing consultant practitioner and foundation consultant practitioner to provide senior clinical oversight seven days a week. The Urgent Community Response Team (UCR) is a rapid assessment, diagnostic and treatment service to reduce unscheduled admissions for people with complex health needs, exacerbation of long-term conditions and frailty. Patients referred to the service are triaged as to be seen 'within 2 hours', or 'same day' for an urgent assessment. The H@H Frailty Virtual Ward supports patients who would otherwise be in hospital to receive the acute care and treatment in their own home, which prevents them from deterioration that would require an admission. The UCR and Frailty Hospital at Home Virtual Ward team operates from multiple bases across Mid and North Hampshire. The service operates 0800hrs-2000 7 days a week, 365 days a year.
