Musters Medical Practice is a well-established, friendly and high achieving practice, consisting of 9,700 patients with a multi-disciplinary nursing team consisting of HCAs, NAs, Practice Nurses, working to provide high quality patient care. Due to retirement of a long standing and much valued nurse, we are looking for a motivated Practice Nurse to provide care for the presenting patient, treatment, chronic disease management and evaluation of care. Working in accordance with the NMC Code of Conduct, the post-holder will require experience in cervical cytology and baby immunisations essential. The post will play a crucial role in planning, co-ordinating the work of the nursing team under the guidance of the Lead Nurse. To develop, implement and embed health promotion and wellbeing programmes To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health To maintain accurate clinical records in conjunction with current legislation To ensure read codes are used effectively by all members of the nursing team To ensure the appropriate maintenance and use of chronic disease registers To implement vaccination programmes for adults and children To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual) To support the clinical team with all safeguarding matters, in accordance with local and national policies Provide care for minor injuries, dressings, and higher-level wound care, liaising directly, where appropriate with community and secondary care counterparts, escalating to a GP or referring onto community services e.g., leg ulcer clinic/tissue viability etc. Undertake opportunistic screening when clinically required e.g., blood pressure, BMI, smoking status, alcohol intake etc. Perform venepuncture and electrocardiography and suture and staple removal Assessing problems presented opportunistically by patients "}
