North East Derbyshire Primary Care Network Ltd (NED PCN) has an exciting opportunity to join our PCN team. At NED PCN we already employ Home Visiting Clinicians who work to care for patients across our four member practices. Existing team members have backgrounds as Community Matrons and Paramedics. The Clinicians play an integral role within our general practice teams, which is why we are looking to expand our PCN workforce. The role will involve developing continuity relationships with our housebound and care home patients, taking a holistic approach to their management.It will involve true MDT working, building relationships with GP practices, Care Homes, Community Rapid Response services and Falls services, to help clinically deliver the service on a day-to-day basis. This is an evolving service, so you will have the opportunity to contribute to the development of the Home Visiting Service if you wish to. The postholder will act autonomously within their professional scope of practice providing care for patients face to face, telephone consultations and home visits. The role includes the provision of weekly planned care home ward rounds. The postholder will operate within their own sphere of competence and agreed acceptable limits of practice to provide expert professional clinical advice to patients, carers and colleagues, ensuring clinical safety and excellence. The practitioner will need to prioritise the needs of patients, making any necessary referrals for investigations in the appropriate manner. The practitioner will be supported in their role by a Community GP and also GPs within the practices within the PCN. There will be opportunities for upskilling and further training within the role to meet the needs of the service if necessary. Job share and part time applications are also welcome.
