You will hold an individual caseload, working as a Care and Treatment Co-ordinator and contribute to improving the lives of young people experiencing mental distress. As we progress our service improvement program within Specialist-CAMHS we see this important post as central to transform the way we provide support and treatment for children /young people and families. Furthermore, we offer the opportunity for further learning and professional development within HDUHB with robust supervision and close links to local Universities to promote career progression. Applications are invited from highly motivated and experienced nurses with practical and theoretical knowledge of mental health assessment and who are able to offer evidence based therapeutic interventions for young people with Specialist-CAMHS presentations. They will be providing psychological based therapies under the CAPA model undertaking Choice and partnership sessions to Children and Adolescents who have been accepted into Specialist-CAMHS services. The successful candidate will work under the Mental Health measures and therefore will work as care co-ordinator and undertake the Care and Treatment Plan (CTP) role of those young people who require ongoing support under Part 2 of the Mental Health measure. The role will require the ability to work on an individual basis whilst being an integral part of the wider multi-disciplinary team. As a senior practitioner within the team, you will be responsible for managing individual caseloads and maintaining clinical records and information in relation to workload. The role will involve supporting and supervision of HCSW in providing and developing care packages for individual service users and their families. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
