We are looking for a a flexible, self-motivated person, willing to embrace change and be proactive in continuously improving service delivery. The post for up to 20 hours per week. Although the main responsibilities follow the PCN Direct Enhanced Service (DES), we envisage the right candidate will also help in projects wider than the DES. The hours of work will be be an agreement within the GMS core business hours 8:00 - 18:30h The post holder will work on a rota basis from our PCN hub in the new purpose built Mixenden Community Hub and from practice premises, and for that the candidate will be required to travel independently between practice sites. Occasionally, there may be the need to perform house visits to patients in their homes. All members of staff should be prepared to take on additional duties or relinquish existing duties in order to maintain the efficient running of the organisation. This list may be amended, following consultation with the post-holder, to facilitate the development of the role, the Network and the individual. Main Duties: An essential part of this role will be focussing on the holistic review of patients with Learning Disabilities aged 14 and over. The PCN has a well established and successful process for conducting the reviews from practices and patients' homes and we are looking to continue this process whilst being open to suggestions for improvements, whilst maintaining wider collaboration with other system partners to ensure we provide the best care and support. Another established workflow is to take part in Enhanced Health in Care Homes part of DES contract, which entails preparing and updating Personalised Care and Support Plans for care home residents. In addition, there may be a requirement to provide leadership and mentorship to trainee Nursing Associates within the PCN, and you will work with ourPCN PracticeNurses to support with the development, rollout and running of PCN wide projects and services.
