Act as the clinical expert and professional resource to the patients and carers and multi-professional team, within their specialist service Work alongside other Specialist nurses/Matron to ensure the development and delivery of a high-quality service Directly or indirectly influence all aspects of care and management of patients within the service/specialty. Deliver high quality, clinically effective care, and patient experience, utilising research and in collaboration with colleagues. Work with the multi professional team to manage the care of our patients with diabetes to provide seamless care. Manage own delegated caseload of patients, as appropriate within the specialty, delivering individualised and personalised direct patient care, whilst facilitating the development of self-care and knowledge.
