To perform as an autonomous general practitioner working independently and with other health care professionals to assess, diagnose and treat the conditions of patients attending within a primary care setting. This role will be mainly based in care homes across the PCN to help deliver the Care Home DES. This would involve supporting the Care Home teams already in place, supporting the Care Home GPs with ward rounds/MDTs, proactive care planning and some acute care. The role may also involve supporting the Physician Associate with supervision and prescribing. About us Key Duties and Responsibilities To perform as an autonomous general practitioner working independently and with other health care professionals to assess, diagnose and treat the conditions of patients attending within a primary care setting. Provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence. To be involved in the provision of education and training to GP ST3s and other staff. To be professionally accountable and responsible for all elements of clinical practice in accordance with the GMC code of conduct ensuring the safety of patients and quality of their care is primary. Providing counselling and health education Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate Recording clear and contemporaneous consultation notes to agreed standards Collecting data for audit purposes Compiling and issuing computer-generated acute and repeat prescriptions in a timely manner Prescribing in accordance with the ICB prescribing formulary whenever this is clinically appropriate Effective management of long-term conditions Ensure compliance with the appraisal and revalidation process Commit to self-learning and instil an ethos of continuing professional development across the practice team Encourage collaborative working, liaising with all staff regularly, promoting a culture of continuous improvement at all times "}
