You will work as one of our two Cornwall-based Practitioners. The Outreach Service covers three Counties; Devon, Somerset and Cornwall working with admission vulnerable women in the perinatal period. We work alongside Community Perinatal Mental Health Service as well and the Mother and Baby Unit (MBU, Jasmine Lodge). Outreach supports and facilitates admission and discharge into and out of the MBU and provides intensive community support for women experiencing acute mental illness in the perinatal period. You will be well supported to develop your skills both clinically and professionally and will work alongside a supportive and cohesive Team of experienced band 7 Clinicians. Holding a caseload of complex women who are admission vulnerable but also women who have been discharged from an MBU following a period of acute illness. Work alongside the rest of the perinatal pathway including Community Perinatal Mental Health Teams and the MBU itself. Complete complex assessments and offer clinical intervention in the community. Support and lead on safeguarding processes for the families you work with whilst also holding the baby in mind as a priority throughout your clinical work. Work closely with the families and partners of the women on your caseload. Engage in complex liaison with other services. About us Communication and Working Relationship Skills Work closely with key internal and external personnel and stakeholders including midwifery, acute medical staffing, Social Services provision, Housing, Voluntary and non-statutory agencies and a wider Health and Social Care Community. Co-ordinate and investigate complaints, ensuring that any clinical lessons learned are identified and disseminated to the relevant teams. Ensure that any learning from SUIs and RCAs is identified, disseminated and where necessary put into practice. Ensure close liaison with primary health care staff and commissioning officers regarding referral criteria into the services and quality standards and ensure targets from the National Framework are met. Liaise with professional stakeholder groups and interface service providers and agencies to develop mutually helpful working relationships.
