We are looking for an enthusiastic pharmacist to support our community services whilst we merge and become a new organisation with Kingston Hospital NHS Foundation Trust. Responsible for the delivery of clinical pharmacy services to the Community Teams within Hounslow and Richmond Community Healthcare Trust (HRCH) and in line with relevant Service Level Agreements agreed with the contracted community provider Your Healthcare CIC (YH) Be ward based at Teddington Memorial Hospital, working in close association with GPs, Clinical Nurse Leads, Quality and Risk Leads, Medicines Management Lead and the Controlled Drug Accountable Officer- to provide and develop safe and effective systems and services across the organisation. Provide professional pharmaceutical support to multidisciplinary community teams across Richmond and Kingston localities. Support the Urgent Treatment Centre, Outpatient Departments and Rapid Response Teams at Teddington Memorial Hospital as directed by the senior pharmacist. "}
