A good standard of English and Maths (GCSE or equivalent) and NVQ Level 3 (or equivalent) is required, along with excellent communication skills. Access to and ability to use transport within the Swadlincote locality is essential. You will work as part of a wider integrated team providing holistic care to patients in their own homes and care home settings. Community HCA's carry out a plethora of patient care including wound care, catheter care and administration of injections. Phone call enquiries welcome, informal visits can be arranged. This post will involve working within a community nursing team visiting patients within their own homes. The successful applicant will: Assess, implement and evaluate individualised patient care in the most efficient and clinically effective way whilst maintaining the patient's privacy and dignity. Provide the delivery of high-quality care and actively support the District Nurse in the management of the health care environment. Work in close collaboration with the Integrated Community Teams. "}
