Specialist Community Public Health Nurse - Health Visitor 0-19 Universal Children's Service . West Locality Team is going through some exciting changes supporting the development of the family/community hubs in Barking . This role is Specialist Community Public Health Nurses - Health Visitors on the NMC register Job Overview To lead the delivery of the Healthy Child Programme, and ensure the Service model is implemented in practice. Ensure services are provided within the 4 tier model- Your Community, Universal, Universal plus and Partnership plus. This will incorporate building community capacity, and embedding this into caseload, team and locality management. Responsible for the management of caseload allocation as per locality/team, and classification of cases in line with the national strategy of Universal, Universal plus and Universal partnership plus. Work in accordance to the Locality commissioned service specification, to ensure the 5 mandated reviews are completed within required timescales and services provided are evidence based practice.
