About us Pharmacist Berkshire Independent Hospital - Reading Full Time 37.5 Hours Job Description Participate in the clinical ward pharmacy service covering the inpatient wards and outpatients To deliver a clinical pharmacy service to the defined clinical areas which will include safely and efficiently undertaking: drug history taking, medication reconciliation, utilisation of patients own drugs, self-medication schemes, discharge counselling, medicines optimisation. Additionally supporting / advising the ward on medication safe and secure storage and handling. Make appropriate referrals where necessary; ensure the clear and effective communication of patients' pharmaceutical needs to other pharmacists and health care professionals both within the trust and in other health care environments. Provide prescription review and medicines-related advice to healthcare professionals at ward level in a range of acute specialties, with advice from Senior Pharmacists where required. Provide timely and accurate advice on prescribing issues and evidence-based prescribing Identify and resolve (with advice from Senior Pharmacists where required) prescribing-related problems to ensure the principles of clinical governance and the safe and effective use of medicines are followed.
