Are you a skilled and compassionate Pharmacist ready to play a crucial role in patient care? Join our team at Oxford University Hospitals NHS Foundation Trust and help us make a difference! Work as part of a multidisciplinary antimicrobial stewardship (AMS) team to deliver the AMS agenda. On a patient level, you will support clinicians in the safe prescribing of antimicrobials through ward rounds, responding to ad-hoc queries and proactive surveillance. With the support of the AMS team, you will develop expert knowledge in antimicrobials and infection management such as the pharmacokinetics, pharmacodynamics, spectrum of activity, safety profile and monitoring requirements of antimicrobials. Optimise transfer of patient care, though timely completion of medicines reconciliation and communication with GPs and community pharmacy teams. Accurately and sensitively communicate medicines related information to a variety of healthcare professionals and patients including those with language difficulties, physical or mental disabilities, in a way that facilitates shared decision making. Support OUH dispensaries, and liaise with external agencies (Such as aseptic production services or homecare providers) to ensure safe, timely and appropriate supply of medications to patients.
