The role of the enhanced nurse will be to work autonomously within practices and community, using enhanced clinical assessment and treatment skills to provide nurse led services, including leg ulcer clinics, home access services and long-term condition reviews. This position will involve regular travel to a variety of sites across the PCN. Consult with patients, their family and multi-professional team to undertake assessment of patient need and devise and evaluate complex care plans. Deliver enhanced clinical care within the leg ulcer level one service. Provide an holistic lower leg assessment including dopplers and the ability to liaise with the level 2 services and TVNs. Provide enhanced care for the home access team, working autonomously to provide long term condition reviews to the house bounds and care homes, meeting the needs of the patient and family. Participate in clinical audits and research projects and implements changes as required, including development, and updating of practice protocols/guidelines and procedures locally. Work within national and local protocols. Delegate work to other members of the MDT and takes responsibility for the delegated activity as appropriate. Provide professional and clinical leadership and mentorship, acting as a role model on the delivery of evidenced based practice. Provide mentorship to pre-registration nurses and is willing to play an active part in the needs of the learning organisation. Contribute to the delivery of local policy, procedures and protocols for the PCN and improve assessment, ongoing support for patients and families.
