We are seeking to recruit a Health Care Practitioner specialising in Older People to join our friendly team in the lovely town of Sherborne. You will be joining our existing Frailty Nurse providing a proactive role within the Sherborne PCN MDT, to identify and support the holistic assessment of our elderly patients. Working collaboratively to provide the best care for frail older adults, both living independently and in Care Homes. With a caring and compassionate outlook, you will responsible for delivering effective comprehensive geriatric assessment. Compiling Care Plans, and supporting patients discharged from secondary care. Applications are invited from experienced Healthcare Practitioners who are able to demonstrate enthusiasm and a clear patient centred approach to promoting independence and wellbeing. Although based at The Grove Medical Centre, your role will involve meeting patients in their own homes, care homes, and working from our other two practices: The Apples Medical Centre and Yetminster Health Centre. This is a full-time position, currently Monday to Friday (some flexibility is possible). A current driving licence and vehicle will be required. Working closely with GPs, nursing teams and other staff of the MDT across three practices, and in the community. Supporting vulnerable older adults and their families, acting as the link between the MDT, patients, and their relatives / carers.
