We are looking for experienced band 6 nurses to support and lead our staff and patients alike. Work closely as part of the Multi-Disciplinary Team for the care of patients requiring medical and rehabilitation support, predominantly post an acute hospital stay. Responsible for the assessment of care needs, development and implementation of patient centered care. Promoting independence and safe discharge home. Lead in our Nurse led unit, especially during out of hours shifts. Shift work and shift management, working with junior members of staff in all aspects of patient care, participating in training and support.
