The role will include completing triage and assessments with clients with a range of mental health issues, ensuring clients receive the most appropriate assessment of their needs and on-going referral to treatment/support services. Working collaboratively with locality team staff, to ensure the best outcomes for clients. Your team base will be at St Andrews Place, Hull, and cover Hull and East Riding.
