The role involves working alongside a variety of professionals to improve outcomes for patients. This is a unique and rewarding role which will allow you to work with patients within their usual place of residence to enable them to remain there safely, the role will involve providing all aspects of personal care, including, pressure area care, supporting with nutrition/hydration, continence support and positional changes. Car driver with access to own vehicle is essential for this role. Main Duties of the Role To work independently delivering care to patients within their own home, following assessment by a clinician. To support patients with hydration and nutritional support. To support patients with continence needs and support with positional, manual handling needs. To communication effectively with patients carers, families and other professionals. Maintain accurate and comprehensive documentation both written and electronic. Ability to work as part of a team and independently.
